All referrals for Offer/Youth Scholarships must come from an Officer or a Youth Liaison. If you are a leader of an organization in the City of Upland and would like to become a Youth Liaison, you may fill out the "Youth Liaison Application" PDF form (below) and mail or scan/email it as directed on the form.
After receiving your application, a Foundation Board Member will contact you. Approved Liaisons are given training on how to fill out the Scholarship Recommendation Forms for submission. Liaisons are assigned an Officer who will receive all of the youth recommendations from that Liaison and be available to answer questions.
Officer approved scholarships are sent to the Foundation Youth Director who writes the congratulations letter to the parents/guardian and youth. The registration and/or equipment purchase is then funded.
